Documentation establishing the presence of a disability is the first step in legally obtaining higher education accommodations, and it helps us work together to determine academic accommodations that will work best for you during your time at 51ÊÓÆµ. Accommodations listed within the documentation will be considered as recommendations to help guide the interactive process.
Students must submit one form of documentation from our established guidelines which best aligns with their disability.
The types of documentation our office accepts can be found on our"Disability Documentation Guidance Frequently Asked Questions (FAQ)" section below. Documentation must be submitted using one of listed forms and also meet the minimum guidelines below:
- Documentations must be appropriate to your disability
- Documentation must be dated within the past 5 years
- Documentation must be on a non-editable format.
- All documentation must be on official letterhead, dated and signed
